We provide outpatient psychiatric evaluation, psychiatric medication management, medication reconciliation, transitional psychiatric care, perinatal mental health services, and ongoing psychiatric treatment for clinically appropriate patients.
For patients transitioning from psychiatric hospitalization or another higher level of behavioral health care, our Psychiatric Transitional Care Program prioritizes continuity between discharge and outpatient psychiatric treatment.

The transition following psychiatric hospitalization or another higher level of behavioral health treatment is an important period in a patient's recovery.
Our Psychiatric Transitional Care Program provides outpatient psychiatric evaluation, medication reconciliation, medication management, and continued treatment planning for clinically appropriate patients transitioning back into the community.
For accepted post-discharge referrals, we prioritize timely access and strive to provide an appropriate psychiatric follow-up appointment within 7 calendar days of discharge whenever clinically appropriate and scheduling capacity permits.
